Guidance Notes

If you are not able to work your shift then you must follow the below procedure. 


Between the hours of 7am - 11pm you must call the Emergency On Call phone number, 01978 851 576 until you speak with someone in person.


Between the hours of 11pm - 7am, you must complete the form on this page and explain in detail the reason you are not able to work along with the Service Users name, date and shift times you were due to work.

The only instance in which you should make a phone call between these times is to report a Notifiable Incident, which are detailed on the attached 'On Call document' below.


Once you have completed the form, you must also call the Emergency On Call phone number at 7am until you speak in person with the person On Call.


If when you call, no one answers, you must keep calling back until someone does answer as they may be busy dealing with other on call issues.


Please also be aware that there may be a slight delay in hearing the ring tone when calling so please wait until you hear the ring tone. This is due to the phone system we use.
